Create a poll with as many questions as desired

This customizable app lets you create and administer a poll of virtually any length, comprised of any combination of fill-in-the-blank and multiple choice questions. The Administrator link permits you to view summary data for any or all questions, to see the detailed responses of each user to any or all questions, and to see correlated data for the first ten questions.
